ABOUT THE ROLE:

The RN Clinical Liaison is responsible for the recruitment and assessment of potential residents for the Health Center. The Nurse Liaison manages the Health Center admissions process and maintains relationships with physicians, discharge planners, and other care providers as well as prospective residents/guests and their families.

YOU ARE EXCITED ABOUT THIS OPPORTUNITY BECAUSE YOU WILL…

  • Assesses potential residents/guests, reviewing their medical records, and determines whether they should be admitted for care in collaboration with physicians, nurses, therapists, social workers, case managers and other interdisciplinary team members.
  • Manages the health center’s public relations to include health fairs, screenings, center education programs (CEU’s), and outreach.
  • Participate in local organizations identified as bringing value, by making presentations and joining as a member where appropriate, to facilitate a broader knowledge of the industry and of the health care center’s capabilities.
  • Reviews all admission related paperwork with the resident and or/responsible party including financial data, signatures, pertinent social information, and medical information, attending physician information, Medicare Part A coverage and non-coverage procedures, and other pertinent and required documentation.
  • Informs the Medical Records/Administrative Assistant of on-going daily census activity such as admissions, discharges, deaths, room transfers, changes in payment methods, and temporary and permanent apartment status.
  • Maintains complete confidentiality regarding admission information and resident records.
  • Assists in coordinating admissions/transfers/discharges with other departments.

WE ARE EXCITED ABOUT YOU BECAUSE YOU…

  • Graduate of an accredited school of nursing. RN with a current license in Pennsylvania.
  • Two years’ experience in long-term care nursing with strong leadership skills required. Participation in professional organizations, dealing primarily in geriatrics preferred.
  • Good working knowledge of Medicare Part A, certification procedures, and medical terminology.
  • Basic computer proficiency.
  • Can generate a warm, friendly, and caring manner on first impression.
  • Can communicate effectively with residents, families, staff, and visitors.
  • Maintain the highest level of confidentiality.
  • Can pass a pre-employment drug screen and criminal background check.
